NOMINATE A 2013 PUBLIC HEALTH HERO
Criteria: A nominee (either an individual or an organization) may have had a significant influence on important legislative health policy issues; prevented the spread of a disease or contributing conditions; been a public policy advocate; aided an at-risk or vulnerable population faced with serious threats to their health; acted to safeguard or improve the environment; served with distinction as a mentor or teacher; or made a significant biotechnological, drug, or diagnostic discovery. Current School of Public Health staff, students, faculty, emeriti, and affiliated organizations, such as centers, are not eligible for nomination. Alumni may be nominated.
To submit a nomination: Please write a one-page nomination letter citing accomplishments, exemplary service, academic background or association, and criteria that characterize the individual or organization as a Public Health Hero. Be sure to include the nominee’s contact information and organizational affiliation (if any), and your name and contact information, including mailing address and e-mail address.
Send the letter as an attachment by e-mail to heroes@berkeley.edu by May 25, 2012.
